VA Consent Form and FAQ

Please complete the form in the link below regarding your eligibility to participate in clinical training at the Veteran’s Affairs Health System (VA). This is required of all students. Students may decline to consent but they still need to submit a response via the form. Please see the attached FAQ. If you still have questions, please contact Dr. Suzanne Allen: suzaalle@uw.edu

UWSOM Consent and Release of Information –  Veteran’s Affairs Health System (VA)

In order to be considered for a rotation at the VA, the School will need to release to the VA your current gender and country of citizenship. This is information the School has not previously collected from you, so we need to collect it from you and obtain your consent to release it to the VA.

In addition to the above information, the VA also needs the SOM to verify your Selective Service status.  In order to be eligible for a clerkship at the VA, all male students need to be registered with the Selective Service. Visit https://www.sss.gov/Home/Registration to register for the Selective Service or to check your status.

Please complete the required form by Wednesday, September 8, 2021.

Financial Aid Update

Your aid should arrive in your bank account on Friday, September 3.

  • Wyoming, Alaska, Montana, Idaho students: An individual disbursement cannot exceed $15K. If your Fall Quarter disbursement is more than $15K, Student Fiscal Services (SFS) will place a temporary tuition charge on your account. Once your first disbursement has processed, they will remove the temporary tuition charge so you can receive your second disbursement.
  • Seattle/Spokane Students: Ensure you are registered for classes or you won’t receive aid.

Career Advising Student Advisory Board – Tip of the Month

Welcome!! Every month you’ll get a tip in this newsletter to help you stay on track from a career advising standpoint. You’ve gotten (or will soon get) tons of emails about extracurriculars! This may seem overwhelming during MCBD but think big picture about what might interest you career-wise. If you can, try to take on a leadership position for an interest group, volunteer project, or committee!  Make sure you don’t overcommit but picking 1-2 extracurriculars this year to pour into is a fantastic way to kick off med school! Pathway applications will also open this fall and can be helpful in learning how to better serve minoritized communities!

In case you missed it:

Here is a link to last week’s Student Newsletter (8-26-21). That issue included the following information:

  • Exemption from in person learning for MCBD and Research Methods
  • Reminder 2021 AAMC Matriculating Student Questionnaire (MSQ)
  • Flu Shots due by October 1
