Reporting concerns in the learning environment:

The Learning Environment Reference Card is a handy tool that describes the procedure for reporting concerns in the learning environment as well as other important information. It includes a QR code for quick access to one of the reporting mechanisms, the online Learning Environment Reporting Tool (aLERT). If you don’t already have a card, here’s how to get one:

  • Students based in Seattle or with upcoming rotations in the Seattle area ‐ Cards are available both inside and just outside the medical student lounge located on the 5th floor of the Health Sciences Building. (Look for a manila envelope.)
  • Students at a Foundations site besides Seattle – Your Foundations dean can provide you with a card.
  • Students located elsewhere – Please email Melinda Frank, Director of the Learning Environment, at mmfrank@uw.edu.

Add the aLERT favicon to your home screen:

  • Enter the aLERT URL: https://aaqt.som.uw.edu/
  • Android – click on the ellipsis at the top right or bottom corner to add to home screen
  • iPhone – click on the share button at the bottom to add to home screen
  • If not already auto populated use “report concern” as the favicon name
