UW IDEAL-LOGIC SEXUAL MISCONDUCT DECLARATION FORM OVERVIEW

The University of Washington is committed to ensuring a safe environment for all members of the UW community. As a part of that commitment, and in compliance with Washington state law*, a final candidate for a UW academic appointment must go through a sexual misconduct disclosure process before receiving an offer of employment/appointment.

Effective October 1, 2020, the University of Washington is required to ask final candidates to sign a statement and disclose information regarding
current or past sexual misconduct investigations and/or findings. The University of Washington cannot hire an external applicant who has not signed
the statement for any academic personnel positions.

*On June 11, 2020, the Washington State Legislature passed House Bill 2327, addressing sexual misconduct at post secondary educational institutions,
preventing employees with a history of undisclosed sexual misconduct, to move between institutions. This bill includes a number of requirements that
the University of Washington must phase-in over the next three years.
