Education Verification Paperwork

What you need depends on where you matched.

In Washington state

• UW SOM Registration sends the Washington Medical Commission a Letter of Expected Graduation.​
• Complete the Consent to Release Certification of Expected Graduation before we can send the letter.​

Outside of Washington State

• Make sure to fill out applicant information and/or signed release if required
• Complete the Residency Paperwork Request Form and send required documentation to somreg@uw.edu
