What you need depends on where you matched.

In Washington state

• Complete the Consent to Release Certification of Expected Graduation to request a Letter of Expected Graduation be sent to the Washington Medical Commission

Outside of Washington State

• Complete the Residency Paperwork Request Form and send requested documentation to somreg@uw.edu
