Note for MS1s and other students in Puget Sound: COVID-19 vaccine booster required at UW Medicine Seattle sites (UWMC, Harborview, Northwest Hospital)
UW Medicine facilities require that students have a documented COVID booster. If you have not yet received a COVID booster, you must get a booster before you will be allowed to do clinical work at a UW Medicine facility. This includes clerkships, shadowing, preceptorships, FCM and PCP at UW Medicine hospitals and clinics.
If you have received your COVID booster, please contact the Health Sciences Immunization Program (HSIP) at myshots@uw.edu to request they create a to-do action in your Castlebranch UX54 To Do List so you can upload the documentation. Documentation must include:
- Name
- Date of vaccination
- Vaccine manufacturer
- The source of the record (clinic/pharmacy, provider signature, or verified registry document)
