Health Provider Contact Information

• Create a Health Provider Contact Sheet for your student.
• The sheet should have contact information for a healthcare provider that the student may utilize for their personal healthcare needs.
• Must be a care provider who is not involved in the student’s educational experience and either be available at your clinic site or another clinic in the community.
• If possible, an agreement to provide care for a student should be made by the site director with the designated provider prior to the arrival of the student.
• Would be very helpful to have the networks and a list of insurances that are accepted by the designated provider.
• This information sheet should be given to the student at orientation.
