Action requested – Submit COVID-19 Vaccine Booster Records

In an effort to keep your CastleBranch account up to date, we are asking students who have already received a COVID-19 booster to contact UW Health Sciences Immunization Program (HSIP) at and request a COVID-19 renewal action be populated so you can upload your COVID-19 booster dose. When emailing HSIP, please include the following information:

  • Student name
  • Student ID#
  • That you are requesting a COVID-19 renewal be populated in your CastleBranch account to upload your COVID-19 booster dose

If you have not received a COVID-19 booster vaccine, we strongly encourage you to do so. Below is a chart indicating eligibility details, and see the CDC COVID vaccine and booster website for more information.


Previous COVID-19 Vaccination Recommended Booster CDC Spacing Guidelines
Pfizer mRNA booster of either Pfizer or Moderna Booster dose 5 months after 2nd Pfizer dose
Moderna mRNA booster of either Moderna or Pfizer Booster dose 5 months after 2nd Moderna dose
Johnson & Johnson mRNA booster of either Pfizer or Moderna recommended


If an mRNA vaccine is contraindicated a J&J booster is then recommended

mRNA booster dose 2 months after J&J vaccination


A second J&J vaccine dose 2 months after the first dose

If COVID-19 booster vaccines become an HSIP requirement in the future, students in HSIP-participating schools/programs will be notified.  At that time, anyone who has not already submitted a COVID-19 booster dose to their CastleBranch account would be required to do so.
