Prior to the start of Orientation at your Foundations site, there are a series of tasks that you will need to complete to prepare you before you start Orientation and the curriculum. We recommend that you start on this check list as soon as possible, as some of the items may take a few weeks to fully complete (i.e. immunizations and disability accommodations). There is a contacts list on the bottom of the page if you have specific questions about any of the items.
Immediately After Acceptance

- Respond promptly to UWSOM Admissions correspondence and requests in order to complete the acceptance process
- Receive your student number and Private Access Code via email from the University of Washington registrar
- Submit your final official transcript(s) if applicable
- Complete the Fun Facts survey available in the acceptance contract
- Set up your UW accounts and verify your technology needs
- Establish your UW NetID
- Set up a UW Office 365 email account and select the “Forward to UW Office 365” option
- Verify that you meet the School of Medicine’s Computer Requirements
- Complete the E-24 Compliance and Immunizations Requirements
- Review your Financial Aid package
- Request disability accommodations through the Disability Resources for Students office, if applicable
- All accommodations are processed through UW DRS, irrespective of your local Foundations site
- The accommodation application, review, and approval process takes 4-6 weeks before accommodations can be implemented by the School
- If you need testing or other accommodations during medical school, contact UW DRS as soon as possible to ensure these are in place before school begins
- Respond promptly to UWSOM Admissions correspondence and requests in order to complete the acceptance process
Prior to the Start of Orientation

- Secure health insurance
- Depending on your Foundations site, health insurance may be a requirement
- Obtain disability insurance
- Indicate a preferred name
- Preferred names will appear in various School of Medicine and University of Washington rosters and databases
- Review and agree to the following policies:
- Complete the following forms (the forms will be available in late June):
- Consent to Release Records Form
- UW Medicine Publicity Release Form
- UWSOM Photo Release Form
- Baseline Data Collection and WPCI Longitudinal Surveys – these surveys will be emailed directly to your UW email accounts in July prior to the start of orientation
- Complete the following required modules:
- Academic Learning Technology Orientation Module (available in late June)
- Bloodborne Pathogen Overview Video
- Title IX: Husky Prevention & Response (60-90 minutes)
- Review and read required pre-orientation curriculum materials (available in June)
- Check your UW email regularly; the volume will increase starting in summer
- Secure health insurance

Please note that the university at your Foundations site might have additional requirements to complete.
