WWAMI Faculty Appointments

FACULTY APPOINTMENT PROCESS

STEP 1:

THERE ARE FOUR FORMS THAT ARE REQUIRED TO BEGIN A FACULTY APPOINTMENT, TWO OF THESE FORMS NEED TO BE DOWNLOADED TO YOUR COMPUTER IN ORDER TO UTILIZE THE PDF DIGITAL SIGNATURE FIELD:

1. Criminal Conviction Disclosure Form

2. Washington State Patrol Form

Form 1: Curriculum Vitae (CV)

The next 2 forms are used to initiate a criminal conviction background check and are required for Clinical Faculty Appointments, optional for Affiliate Faculty Appointments:

Form 4: Washington State Patrol Form 2.

A-Check Global is the consumer reporting agency vendor that conducts background checks for the university. You will receive an email from them as your appointment is nearing completion. Please be sure to open the email and complete it, as it will expire in 3 days after it is received.

After you have completed step 1 above, please select your appropriate location; an email window will open and forward these documents to your local WWAMI Clinical Office to begin the appointment process:

STEP 2:

 After the above is completed, you will be sent the Sexual Misconduct Declaration Form to complete via Ideal-Logic electronically.

The University of Washington is committed to ensuring a safe environment for all members of the UW community. As a part of that commitment, and in compliance with Washington state law, a candidate for a UW academic appointment must go through a sexual misconduct disclosure process (“disclosure process”) before receiving an offer of employment/appointment.

A candidate who does not hold a current UW position/appointment, must complete the Sexual Misconduct Declaration which includes providing information related to all current and past employment/appointment at postsecondary educational institutions and disclosing any substantiated findings of sexual misconduct by associations with which the candidate has or had a professional relationship, per RCW 28B.112.040. This information is gathered through UW’s Ideal-Logic Sexual Misconduct Disclosure tool and will be used to determine whether a sexual misconduct verification is required.

Below is an example of the email.

Ideal-Logic sends the candidate an email prompting them to:

  • Create an account (if they do not already have one).
  • Answer the SMD questions.
  • Provide information related to all current and past employment/appointment at post-secondary educational institutions.

Ideal-Logic sends a reminder to candidates who do not complete the SMD questions after three (3) days and then again after six (6) days. Units may not move forward with the appointment process until the form has been completed.

This is a required step in the faculty appointment process. If a candidate does not complete this electronic form, we are unable to move their application forward.
